In addition to complying with the College's general
admission procedures, LPNs applying for the Transition Program must:
- Have successfully completed one year of high
school chemistry (applicants may take CHM 110S as an equivalent
course); Note: to take Anatomy and Physiology I and II (BIO 125S and
126S) chemistry must be taken within the last five years or
department permission for bypass given.
- Have satisfactory math assessment score and
three years of college preparatory mathematics, or a grade of "C" or
better in MTH 099D, Intermediate Algebra II.
- Have satisfactory assessment scores in reading
and writing to be ready to take Composition I (ENG 121S) or
successfully complete Basic Writing (ENG 091D) and College
Preparatory Reading (ENG 093D).
- Submit LPN/ADN Transition Program Application
to Nursing Office (available at Nursing office, Room 312, or
online).
- Have official transcript from LPN program sent
directly to Enrollment Services.
- Have been employed as an LPN for six months or
more;
- Have successfully completed prerequisite
courses as follows: BIO 125S and BIO 126C with a grade of "C"
or better; PSY 121S ENG 121S, and CHM 111S. (Note: high school
chemistry or CHM 110S is a prerequisite for BIO 125S);
- Show current Ohio LPN license and NAPNES,
OOPNES, or Ohio Board of Nursing Medication Card to Nursing Advisor.
Applicants selected to enter the program will be
notified by May of the entry year. Space available will determine
the number of qualified LPNs to be admitted up to a maximum of 12 in any
one year. Information is available in the Nursing Office.
Students must complete application requirements
(see above 1-8) prior to the start of the first nursing course.
Those selected for the program are required to have
a physical and medical health form completed by a physician, a current
CPR for the Professional Rescuer card, and a successful background
check.
The first nursing course is online
and requires successful completion of NET100S before enrolling.
